If a parent terminated their rights, is it possible to reverse or repeal the termination?

For instance, one parent is put in to a spot where it is either give up rights or only see their child once maybe twice a month... they were young and going through a rough time and signed over their rights, is it possible to reverse or try to retain parental rights?
